Send a general website inquiry
Please send only the minimum contact and topic information needed to respond. Our general contact form is not a medical intake form or a channel for sending sensitive treatment records. For dental referral inquiries or volunteer listings, use the separate Patient and Dentist Connection Desk.
Patient safety and emergency information
ApplyFreeDental.com is an independent public information directory—we do not provide clinical dental treatment, and we never charge application fees or ask for your Social Security Number (SSN), Medicaid ID, or payment card details. If you are experiencing severe facial swelling, fever, or difficulty breathing/swallowing, call 911 or visit the nearest hospital emergency room immediately.
Our team cannot diagnose an abscess, prescribe medication, authorize surgery or provide emergency referrals. If your need is urgent, contact a licensed dental professional, urgent care provider or emergency service appropriate to your symptoms.
We do not ask for scans, dental radiographs, treatment records, proof of insurance, benefit identifiers, or payment credentials through our general inquiry form. Healthcare providers may request such items through their own secure official intake channels when genuinely required.
How inquiries are handled
Editorial questions are directed toward source review and published-content accuracy. Program listings may be checked against an official clinic or nonprofit contact. Privacy inquiries are routed to our compliance desk. Sending an inquiry does not create a dentist-patient relationship, guarantee a clinical referral, reserve a clinic appointment, or establish entitlement to financial aid.
We take reasonable steps to limit the personal details requested. Our browser-only tools do not transmit screening answers, while a contact inquiry necessarily includes the information you choose to send so that a reply is possible. Please read our Privacy Policy and Medical Disclaimer for more information.
