Agreement to use our independent information service

These Terms and Conditions govern access to ApplyFreeDental.com, a consumer oral health research directory operated by CivicHealth Oral Care Media LLC. By accessing the website or using an informational tool, you agree to these terms to the extent enforceable under applicable law. If you disagree, you should discontinue use of the site.

Our terms were originally adopted on April 12, 2018 and were last revised in October 2026. We may change website features, program references and these terms when necessary. Continued use after a published revision may constitute acceptance where permitted by law.

Informational scope and no doctor-patient relationship

All material on this website, including explanations of charitable dental programs, Medicare or Medicaid benefits, university clinic services, financial hardship letters, eligibility screening and cost estimates, is for general information and consumer education. We are not a dentist, hospital, dental school, government benefits agency, insurance underwriter or charity grant administrator.

Nothing on this site creates a doctor-patient, therapist-patient, insured-insurer, attorney-client, caseworker-client or fiduciary relationship. Our team does not diagnose, examine, prescribe medication, issue medical necessity certifications, provide emergency treatment, approve insurance benefits or determine who a nonprofit must serve.

A licensed dentist or appropriately qualified clinical professional must evaluate oral health, treatment suitability, surgical risks and aftercare. Benefit administrators and individual providers are responsible for their own applications, coverage determinations, patient consent and treatment fees.

Charity availability, 501(c)(3) waiting lists and coverage limitations

References to Donated Dental Services, Remote Area Medical, university clinics, Cosmetic Dentistry Grants, state Medicaid programs or VA benefits describe possible information pathways, not guaranteed services. Each nonprofit or program controls its own current service area, application criteria, patient priorities, volunteer capacity, appointment timing, referral requirements and waiting lists.

Registration as a 501(c)(3) charity does not mean a particular patient is approved or that implants are provided in every jurisdiction. An applicant may be asked to complete clinical clearance, resolve active gum disease or infection, submit qualifying program documents, or wait until a suitable practitioner is available.

Emergency dental needs should not be delayed merely to wait for a charity application decision. If you have severe swelling, fever or breathing/swallowing difficulties, seek urgent medical evaluation instead of relying on this website for triage.

Costs, prerequisite care and patient co-pays

Our articles may discuss donated treatment at $0 for a qualifying case, reduced-fee school or nonprofit appointments, and private cosmetic dentist-network discounts. These are distinct financial arrangements. Actual out-of-pocket amounts depend on the provider, treatment plan, geographic area, covered benefits and any additional procedures.

Examples discussed in our research include no-charge services at eligible DDS or RAM programs, possible preliminary imaging or laboratory expenses, and dental-school or fee-reduction charges that can fall in broad illustrative ranges, such as $250–$1,800. Some referral, laboratory, scan or prerequisite expenses may appear at lower or higher amounts, including examples of $0–$950 in particular assistance descriptions. Neither range is a universal program charge or an offer to treat a particular visitor.

Read the written treatment estimate before agreeing to treatment

Always confirm examination and imaging charges, periodontal care, extractions, sedation, implants, prostheses, follow-up appointments and financing terms. We do not collect application fees and never ask visitors to pay a government dental grant release fee.

Financing, installment plans, promotional interest periods, deferred-interest terms, credit checks and debt obligations are governed by the third-party lender and applicable consumer laws. Our cost tools are planning aids, not financing approval or a quoted total price.

Use of eligibility tools, calculators and referral letter templates

Our browser-based matcher compares general profile, coverage and dental needs to informational program categories. It does not verify your Medicaid enrollment, household income, VA status or clinical diagnosis. The Federal Poverty Guideline estimator uses the inputs you provide and should not replace an official sliding-fee determination by an HRSA-funded health center.

Any letter created through our Medical Necessity & Dental School Financial Hardship Referral Letter Generator is an applicant-prepared educational template. It may assist communication with a qualified professional but does not become a licensed clinician's medical necessity determination merely because it includes your name or a description of functional difficulties.

We grant individuals a free, nonexclusive, personal educational license to copy, edit, print and share their own completed referral letter or hardship statement for a lawful assistance inquiry. You may not falsely present an automatically generated letter as signed or certified by a medical practitioner, forge clinical credentials, or use the template for fraud or impersonation.

Website copyright and permitted use

Original editorial text, website design, internally authored tools, source annotations and layout elements are protected to the extent applicable by copyright and related laws. You may browse, link to, quote brief passages with attribution, and use our downloadable patient letter templates for the personal educational purpose described above.

You may not scrape or republish large portions of our website in a misleading manner, interfere with site security, automate abusive submissions, impersonate our editorial staff, or reproduce third-party trademarks as though you owned them. Names and trademarks belonging to charities, agencies and other providers remain the property of their respective owners.

Information accuracy and limits of responsibility

We strive to review the accuracy of the resources we publish, but programs and policies can change without notice. We do not warrant that every listing is available at a given moment, every link will remain operational, or a particular treatment will be suitable, available, covered or free for an individual reader.

To the extent permitted by law, the website is provided on an informational “as available” basis. Nothing in these terms excludes or restricts liability that cannot lawfully be limited, or overrides non-waivable consumer protections. Clinical advice and decisions made by independent dentists or other providers remain their responsibility.

Responsible submissions and consumer privacy

We do not request Social Security numbers, Medicaid member IDs, payment card information, bank records or complete health histories. Contact forms are intended for brief editorial, program or support communications. Please use the official secure intake process of a treating provider if that provider requires clinical records or identification.

We reserve the right to reject abusive, fraudulent, harmful, off-topic or technically unsafe submissions. Communications may be subject to reasonable operational logging, spam checks and rate limits. See our Privacy Policy for information about website information handling and consumer rights requests.

Governing law: State of Delaware, United States

To the extent legally permitted, these Terms and Conditions are governed by the laws of the State of Delaware, United States, without regard to conflicts-of-law principles. This clause does not prevent consumers from exercising mandatory rights and protections provided by laws applicable to their residence or to a particular dispute.

If a provision is unenforceable, the remainder of the terms may remain effective where allowed. A failure to enforce a specific provision does not automatically waive a subsequent right to enforce that provision.
